Medical errors continue to rise and are one of the top three patient safety concerns today. Organizations committed to a culture of safety are more successful in recovering medical errors. Ms. Markwell has identified a crucial strategy in creating a culture of safety and recovering medical errors. I appreciate her stressing the importance of reporting medical device errors. Again, there is great opportunity for improvement in keeping patients safe and nurses are the key.
